In my experience in Italy and the UK you need to sign up for Prime in that country to get free shipping. I have signed up for a free one month trial and then cancelled. I am a Prime member at home, of course.
FYI, you can stream Prime Video while traveling but what you can stream is restricted to that which is available in the country you are in.
Your Amazon account will be recognized on Amazon.it, but as Laurel says, not your Prime membership. There will be a choice available for faster delivery at a modest cost. The pick-up points for packages is more extensive in Italy than the US and include Post Offices, lockers, newstands, and tobacconists.
As was stated, your account will be recognized but Prime won't. However, you might be offered a free month of Prime in Italy. If so, sign up and cancel before the 30 days is up. You'll get the benefits without the charge. I just did that in the UK.
