What is dialing codes for family in US to call us on our cell while we are in Sicily/Italy?

If it's a US phone they would dial the same number they would dial if you were at home in the US and never left.

Yes - if it is your home cell phone that you have with you then calls will go through the same if you are in Italy or the US. No difference, you will also be bothered by telemarketers the same as at home.

If you buy a new SIM card or Italian phone then you get assigned an Italian number. People at home would need to call +39 (Italy's code) then your Italian number. But that's only if you get an Italian phone or if they need to call your hotel or something. For your normal phone, proceed as if you were just next door.

Going the other way, dialling a number from your "foreign" cell phone while the phone is in Italy, I would always dial with the country code, +1 for the USA, +39 for Italy etc. If you just do area code + number, it could assume you want that area code in Italy or in the USA.
There is no extra charge if you include the country code, but you are then sure. I always save numbers on my phone with the country code.
