Needing help with a combination academically guided tour of Borghese Gallery followed by Rome highlights tour (not to include SC/SPB) on a Sunday in April.
Can you be more specific? By "highlights"? Are you expecting to go inside any of those - such as the Colosseum - or just view them from the street? I assume that SC/SPB means San Clemente and St Peter's Basilica?
Combining the museum + a highlights tour would be a long day on your feet, and you may have to book them separately or have a guide - Francesca Caruso is highly regarded - customize a private tour for you.
I haven't taken it but "Walks of Italy" has a tour of the Borghese. This company consistently receives top marks from RS posters (and reviewers on other sites) who have used them so I'm sure it would be very good. Just an option if you decide to book your tours separately.

Editing to add, hope your Sunday isn't the the first one in April (April 1)? That's "Free Sunday" at the National Museums so while you would still need reservations at the Borghese, I'm pretty sure they won't be accommodating outside tours. WOI's calendar shows no Borghese tours on any First Sundays.
Thank you, Kathy. The SC/SPB early entrance express tour is a new Walks of Italy tour that is for the Sistine Chapel and St. Peter’s Basilica . It is booked for the morning we are leaving the city. Fortunately, our April visit is not the first weekend, but the Walks of Italy tour of Borghese is sold out for our date; therefore I am looking for a reputable guided tour of the Gallery that might include touring city highlights with skip the line access. I found something at Get Your Guide, but they too are sold out; therefore, I am considering a private tour.

Well, hang tight and see what the community might be able to come up with but to do both the Borghese+ skip-the-line highlights with the same guide, I think you're looking at a private, customized tour. It will be expensive but you probably already know that.
You might put "Francesca Caruso" in the search box at the top of the page, hit "go" and then choose results in the forum only. There are a number of questions/firsthand recommendations for good private guides under that topic. From what I understand, Francesca is much in demand and so can be difficult to snag.
