We will be in Palermo May 3 - joining our tour Sunday May 6. We have several family and friends with May birthdays so I would like to purchase and send birthday cards from Palermo. Is this even a custom in Sicily? If so where do I find cards? I tried googling but no useful information came up.

There are stationery stores that you might try. I'm sure I've seen gift wrapping paper in those stores and would assume they also have cards. Also try tobacco shops (tabacchi). If they don't have cards, ask where you might find them.
Send a postcard.
If you do find b-day cards there they likely will be in Italian. Perhaps that is what you want. ?
I would be inclined to buy the cards before departing and mail them from there if if the stamp and postmark are what is your intent.
Or just mail the card before you leave
They will be available in bookstores and stationery stores. Try the term "cartoleria." I'd also try to get stamps and use mail boxes from the national Poste Itlaliane rather than a private mailing service that might advertise to tourists.
