Best Thermal pool experiences in Italy? Tuscany/Umbria/Parma/milan

Hello,
I’m looking for details on the natural thermal pools in Italy.
And reasonably priced small inns/agritourismos nearby. Travelling in April

Anything natural, clean, in a pretty area? Trip advisor reviews say many of the free ones are dirty with litter, etc.

The island of Ischia (near Naples) is well regarded for its thermal baths.

Check Montecatini Terme for a thermal bath.

We will be traveling in Italy in April. I am in the medical field and ask a fellow Infectious Disease MD if she would get into the thermal baths. She stated that she would not which was a deterrent for me. There has been studies of a type of contact dermatitis from the sulfur content and other minerals. I have many allergies and think I may not want to try this after all.... Just a thought.

I did not find the free Terme di Saturnia to be dirty, even though it was busy. There is a pay shower available at the snack bar but I did not check the details of it.

We visited on a day trip from the Orbetello/Porto Ercole area, having stopped first to visit the hill town of Pitigliano.
