I'm interested in an "off the beaten path" tour in Rome. I've seen all of the major sites. I'm interested in neighborhoods, food, street art, and the like. Any tips or suggestions?
In this case, the TI can be extremely useful. We have found some very unusual tours using the local TI.
A friend took a Vespa tour a few years ago, and loved it. Lo, are you online?
It's 30 miles north of Rome, but it's an off-the-beaten path site.
About 30 miles north of Rome is Calcata, which attracts bohemians, hipsters and artists: https://www.withlocals.com/experience/a-day-in-calcata-picturescue-bohemian-town-56806fec/?adults=4&children=0
And a true oddity is that the parish church of Calcata claimed to once house the santissimo prepuzio, the “blessed foreskin” of Jesus, but lost it. Here's the story: https://garymjones.com/travel-calcata-italy/
This website is often good for providing quirky or unusual Rome sites. In this case they offer up 134 things to do/see. I'm sure you will find a few that might be of interest.

Sorry, I just re-read your original post and realized that you were looking for an off the beaten path TOUR not general locations. Can't really help you out with a tour but the Atlas Obscura site is interesting.
It’s definitely off the beaten path. I’d go back to Rome just to scooter around the city. It’s a blast.
Hire one of Rick's suggested guides and let them have their way with you
I've had two friends try the Eating Europe tour in Trastevere which they loved as it's a more bohemian/artsy neck of the woods
