I will be staying in Monterosso al Mare in September. I need to rent a car for about 5 days to travel throughout Tuscany and drop off in Sorrento.
Can anyone recommend the best place to pick up the car?

Look at La Spezia. We picked up our car at Milan MXP, parked it in a Monterosso lot for the three nights in the Cinque Terre, then used it for the rest of out trip. Easier.

If you really want to drive the entire distance, then La Spezia will have the best options. Another option is to train until you're closer to your destination in Tuscany, if you want to avoid driving on the big highways. What is your destination in Tuscany?

The easiest is Avis/Budget which is walkable from the train station but it's not open Saturday or Sunday. Hertz and Europcar are just a short taxi away - they are closed Sat. afternoon and Sunday. All are closed for lunch. However, going direct to the car provider may be cheaper at times and quicker at pickup. If you do not have license from an EU country, you'll need an International Driver's Permit (a translation with a photo) required by Italian law.
