Here is a site showing the various sights in the Phelegraean Fields area. Worth a day or two's exploration. It is some years since we visited these sights but I understand that they are far more visitor friendly eg easier to find and explore, these days.

I guess it depends on how far you're willing to travel and what your interests are. First, if you aren't stopping by there on your way up to Naples, I would definitely recommend going to Capri. Between the towns, the villas, the blue grotto, and the chairlift to the top (and possible hike up to/down from the top), there's plenty you can do and see.
If you're already going to Capri beforehand, then yes, you can try out Ischia. I haven't been there, but heard it can be a worthwhile stop (though not quite as scenic as Capri). Also, if you're interested in the like, I would recommend Caserta (which is about half an hour to 45 min. north of Naples via train) so you can visit the Palace of Caserta. The palace is essentially the Versailles of Italy, so it can make a good visit, again, if you enjoy experiencing such places. I've also heard that Benevento (about an hour and a half outside of Naples via train) can be a town worth visiting.
