I went to the Baths of Caracalla as part of Context Travel's Appian Way tour. It's a pretty pricey tour, but I really enjoyed it. Context limits groups to 6 people and guides are Master's or PhD level "docents." Our docent greatly supported our understanding of what we were seeing and helped us place it in historical context.
FWIW, they have a new virtual reality headset that you can rent. Highly recommended from people who've used it. You can wander around the grounds and see what it looked like at its peak.
