Try La Crociona http://www.lacrociona.com/ about 3km from Montalcino. It is in Rick's book. I have been there and loved it.

Check out II Giglio, it's not a B&B but small apartments and very romantic. It is right in Montalcino so no driving after visiting all of the wonderful wine bars.
