We will be traveling from Monterosso Al Mare to Siena and spending 4 days in Siena, then driving up to Bologna to fly home. Can anyone please suggest the most convenient location to rent a car between Monterosso Al Mare and Siena? What is the best driving route also? Any other suggestions or tips appreciated!

La Spezia, assuming it’s not on Sunday when rental offices in La Spezia are likely closed.
If it’s a Sunday, then you might need to go down to Pisa Airport.

Note that each driver listed on the rental form will require the compulsory International Driver's Permit which is used in conjunction with your home D.L. These are easily obtained at any AAA / CAA office for a small fee and valid for one year.
You'll also have to be extremely careful to avoid the ZTL (limited traffic) areas that exist in many Italian towns these days. Each pass through one will result in hefty fines!

Do you plan to drive around between Siena and Bologna? It is not recommended that you have a car in the cities. If you want a car for driving around Tuscany, recommend renting and returning a car in Siena and taking public transportation to both Siena and Bologna.
