Does anyone know what gates the flights from the US use when the arrive at FCO? Do all of the US arrival flights use the hub terminal and then have to take the shuttle train to the main terminal? Does the shuttle wait for everyone to deplane before leaving for the main terminal. We are flying in on an Alitalia partner flight (reservation is with Delta), and that flight has been running 25-35 minutes late every day. We will be connecting at FCO to our flight to Naples, and I am just trying to get a feel for what we are looking at in making our connection.
The only "satellite" terminal with tram service is Terminal 5. The main terminal complex houses terminals 1, 2 &3. Delta flights arrive in Terminal 3. Your Alitalia flight to Naples will leave from the B gates in Terminal 1. The terminals are connected and, although shuttle buses run between the terminals, the simplest thing to do is walk through the buildings. If your flight does not deplane at a jetway, it will be met by a number of shuttle buses to take passengers to the terminal building. If your trip is a single ticket from your departure airport to Naples and you have checked your luggage, it will be transferred to the connecting flight. You will go through immigration at FCO, customs in Naples. Depending on passenger volume, it can take anywhere from 1 - 3 hours to clear immigration. If you are on a single ticket and miss your flight, Alitalia will put you on the next available flight.
You will arrive at T3 and depart to NAP from T1

I don’t even know why T5 is even mentioned here. T5 was closed several months ago and is undergoing major rebuilding and renovation.
I believe the shuttle train the OP is referring to is the one out to the remote gates of Terminal 3 (gate numbers in the 30's, if memory serves.) If you have used the airside train in Atlanta, it's somewhat similar, though it doesn't have to travel nearly as far. Trains come every few minutes and it's all automatic--the train arrives, the doors open, whoever is on their way out to the departure gates gets off, you'll get on, the doors will close and then you're off to the main Terminal 3 building. So, no, the train does not wait for everyone before it goes. And, yes, you will probably be at those gates, though I'm not clear from your original post if your flight is an actual Delta flight or somebody else. Many East coast flights have been affected by thunderstorms in the past week or so, causing some delays.
