I just rented a car on Expedia for October...reviews are half/half. The price for the economy car was great...but was it too good to be true?

Stick with the major car rental companies Avis, Budget, Hertz, Enterprise, Sixt and book direct or with AutoEurope.com
I would trust only the major rental companies. These are the major companies operating in Italy (not in any particular order, but grouped by holding group).
1 - Europcar
2 - Avis-Budget-Maggiore
3 - Sixt
4 - Hertz-Dollar-Thrifty
5 - Enterprise-Alamo-National-Locauto
6 - Sicily-by-Car
Note that many brands are actually part of the same group. For example Locauto (an Italian brand) has become part of the Enterprise Holdings group (in 2013), just like Alamo-National previously. The same applies to Maggiore, a major Italian rental car brand, which was acquired by Avis-Budget in 2015.
Sicily by Car is an Italian independent company. Not the largest but they don't get bad rating. I had a positive experience with them (in Sicily, I guess in their home turf). Locauto (now part of Enterprise) somehow always got above average ratings, probably the highest in Italy (but I never had the pleasure to try them).
Thank you...after I went back and read reviews...I canceled. Just seemed too sketchy.
