Another COVID-confused traveler

We are going to Italy on May 3. We have had all vaccinations including the booster on October 7, 2021. Are we good to go with our CDC cards? Also what is this locator thing that people are talking about? When and where do we fill this out? Can we do it now? Is it online or is it a paper form? Thanks for helping me understand. Although we have been seasoned travelers, this is the first time we've travelled abroad since COVID.

Check the website Re-Open EU. Use the drop down box to select the country you will visit. It still isn’t easy, but worth a try. I found the locator form here

For vaccinations, that is all you can do. Some talk now about a fourth shot, Moderna has applied to authorize a fourth shot for all, if that applies, Pfizer has only recommended it for very high risk people, so unless something changes, Italy should not impose any additional requirements. There was discussion that a time limit would be set on the booster, but that now will only happen if everyone agrees that a fourth booster is needed. Make sure you bring your original, unlaminated, CDC card, I also suggest getting a printout of your vaccination history from your pharmacy, local health department, doctor, or whoever might have that information. It is not "needed", but good to have in the event you lose you card, or someone has additional questions.

The "locator" form often referred to as the EuPLF, is a form where you provide your flight information, additional personal information, and address of your first stay in Italy. It is intended to provide authorities with more contact information in the event an outbreak was detected on one of your flights. You fill it out electronically, and submit it. Best to fill out a week or so before departure, you need to know seat numbers, and with schedule changes, filling out too early means trying to change it. The form can be found here: https://app.euplf.eu/#/ make sure you fill out the one for Italy, You will get a response, with a QR code, it is basically just confirmation, the airline may ask if you have it, so print a copy, but they will not examine it, no one else will ask for it...it's in the system. After your first night or two in Italy, it is basically obsolete.

Also, as of May 1st they are rolling back all covid restrictions so you won’t need your card to get into museums, restaurants, etc.
