An Afternoon and Evening in Rome

We fly into Rome in mid-September, land at noon, and as we have done several times previously, expect to be at our hotel between Piazza Barberini and Spagna by 2. I have been to Rome at least 8 times (I am losing track) and my companions have been once. On that trip, we did all the major stuff (Ceasar Shuffle, Vatican, Tombs and Catacombs, Veneto and Trastevere etc.) What can we do in one afternoon and one evening that is new, worthwhile off-the-beaten track? Are there streets and neighborhoods with charm, but less traveled. Testaccio, Via Dei Coronari? Is there one must go to restaurant in the Barberini/Spagna area. Avoiding Piazza Navona itself, are the strolls, and sites to see in that general area that won't have us dragging forever. Remember, we are not seeking to see all of Rome in 5 hours, but to use the time to experience something beyond all the usual suspects.

Also, any recommendations for a good quality restaurant, hopefully away from the Duomo, in Orvieto.
