Amalfi coast to tuscany to Lake Como advice

We have 14 days mid October to late October.
Arrive in Rome and go straight to the Amalfi coast and spend 3-4 nights there taking in Capri, Ravello, Positano, using Sorrento as our home base.
We would also like to explore Tuscany and base ourselves in Sienna for 4 nights and then do day trips from there to San Giovanni., Civita di Bagnoregio, Orvieto, etc.
After dropping off our car in Sienna or Florence take the train to Varenna on Lake Como for another 4 nights then fly out from Milan.
I would love to hear your suggestions of places to visit, B&B's or Agritourisimo's, hotels, wineries. We are interested in small out of the way places, romantic get aways, outdoor living, rewarding hikes, views and vista's. We are not interested in crowded cities and will save Rome and most of Florence for another trip.
I am uncertain if it makes more sense to fly into Milan and do the reverse order of what I just laid out and then fly out of Rome.
Do you think we are taking on to big of a trip covering too many miles?
Any suggestions? Do we need a car on the Amalfi coast?
Would it be best to train from Naples straight to Sienna then rent a car to tour around Tuscany?
Any suggestions or thoughts are greatly appreciated.

Given the time of year, it would probably make sense to reverse the order due to your interest in Lake Como. Have no recommendations for this area, but am sure you will get many.

wonderful hosts, easy access to areas you have indicated.

Drop car in Orvieto. Make sure you check hours of drop off.

From Orvieto train to Roma Termini, then train Roma termini-Napoli Centrale. Circumvesuvian to Sorrento.
Favorite in Sorrento (http://www.ilnido.com/) make sure you have one of their dinners. Yum!

Fly out of Naples Lufthsana has a flight that leaves between 12-1 depending on the time of year.
