We will be arriving in Rome and departing from Milan with a party of 6 with six large suitcases. We want to travel by private car/van from Rome airport to a downtown hotel and from Milan Central train station to a hotel next to Malpensa. Please recommend reliable company(s) in Rome and Milan. Also, if anyone knows of a good restaurant within short walking distance of Milan Central Station, that will be helpful as well.

I'll second the suggestion for Romecabs. If people in your party have not been to Rome you might enjoy the VIP transfer which includes a whirlwind tour thru the city. I wound up booking the VIP transfer because I waited too late and they were booked for a regular transfer so I went with the VIP for the 5 of us. I though it might orient us to Rome, haha. While it did NOT do that (too many crazy twists and turns) we did stop a couple of times to overlook Rome and peek thru the Aventine keyhole. We saw a lot and my group enjoyed it! (Well, my brother got a little motion sick because he called shotgun and the weaving thru tiny streets bothered him.)
If you are at Milan Central train station, then you can take the train direct to Malpensa airport for €10 per person. Fixed rate for a taxi is €90 but you'd have to find a larger taxi for your group. Sorry, I don't have a recommendation for a separate shuttle van.
