I am on a 2 pm flight on Sun, 5/5, flying out of MXP. If I decide to stay in Parma on Sat night, what are cons in doing so? Or would you recommend staying in Milan on Sat night? Not sure how reliable trains are going from Parma back to Milan on Sun morning.

The con is missing your flight. We found out the hard way that Italian trains are not as reliable as say Swiss trains. Your choice, though.

I wouldn't chance it. Trains can be delayed. Not a matter of Sunday morning... the problem is if something goes wrong on the lines. I've been delayed more than 2 hours going to Milan from Bologna once this year and once last year. Other times on time or short delays.

Yes.
As you have already considered, train service can be disrupted by any number of things. Go to Milan mid day Saturday, have a good walk about, a good dinner, and not be stressed about making your flight.

I agree, stay in Milan. Plenty to see and do as well as ensure you make your flight. The train to the airport (Malpensa) is 55 minutes. Plan accordingly.
