Additional destination to add to 1 week trip

Traveling to Italy via Rome with another couple in January. My husband and I previously visited Italy two years (went to Milan, Verona, Venice, Florence, & Rome) and are looking for an additional destination or two besides Rome. We were hoping to add somewhere we haven't been, initially thinking the Amalfi Coast. Due to seasonal closures wondering what everyones opinions are additional location to add? We are there 7 nights, planned on spending at least 3 nights in Rome. Since we don't have too much time, were thinking a destination no more than 4 hours away on a high speed train.

Wondering thoughts on Siena, Naples, or smaller towns outside of Tuscany? Figured our friends could always take a day trip to Venice or Florence from one of these destinations? Open to any and all suggestions of places. We won't have a car, so we would like to be near public transportation.

Even in January, Naples gets my vote. While the Amalfi coast might be less enticing at that time of the year, you can still explore the city itself, as well as Pompeii, Herculaneum, Capri even, just using trains and ferries.

In winter I think cities. You would not run out of things to do indoors in Naples, heck if you gt a sunny day you could still see the beauty of the Amalfi coast (though I would not plan a trip around it in January). You could see Pompeii without searing heat, go down to Paestum to see temples.
But then there would not be the option to day trip to Venice and Florence as you mentioned.

For fans of food and cars, Bologna could make a nice city break.

Of all options, I'd opt for a week in Rome.

You could take a daytrip from Rome on the train to Orvieto or Assissi.

I also recommend Naples. Underrated, the city has much to see and do. Easy to take a day trip to Pompeii or Herculaneum.

I spent 5 nights in Salerno to explore the Amalfi Coast in February. Take a high-speed train, stay in Salerno, use the SITA buses to see the coast, they won't be crowded. True, many of the tourist shops and restaurant will be closed in the villages, but the best of the AC is the beauty, and its even better in low season. . . unless you get poor weather, which is possible. You can also visit the Greek temples in Paestum by train or bus. Or arrange a driver and stop on the way at a buffalo farm for fresh mozzarella and the best gelati and canoli ever. Salerno is great in winter, mostly locals and everything is open - shops, restaurants, (most) hotels.

Naples is probably a better choice because it's not as weather-dependent as the AC. A nice day trip is to Caserta.
