Flying out of MPX on a 6:45 flight to Frankfurt then on to Seattle. Looking to stay at a hotel near MPX that offers shuttle service beginning at 5:00 which should arrive at the airport (3.5 km.away) around 5:15. Is this sufficent time to make this flight or should we take a taxi to get us there earlier?

You could stay even in Milan and take a Malpensa Express train. It's a 30 min ride from the Cadorna train station, near the Sforza Castle. http://www.malpensaexpress.it/en/ Malpensa airport is in the middle of nowhere, but if you want to stay there you can do a search on any website and find something right there at the airport, where there are several hotels. Most of them will have a free shuttle to the airport. Since your first flight segment from MXP to FRA is a European intra-Schengen flight, it's considered the same way as a domestic flight here in the US. The requirement is to be there approximately one hour prior to departure.
