We are returning home from Milan Linate airport; our flight leaves at 7am. The hotels in the vicinity of the airport don't get very good reviews and often don't have a shuttle leaving early enough. Would appreciate any advice for a good place to stay, including other areas of the city that might provide better transportation choices.

If you stay near Milan's Stazione Centrale, you can take a bus to Linate. Or, from anywhere else, you can take a taxi (it won't be too expensive, and at that early hour it won't take too long - Linate is close to the city).
Remember that your flight from Linate is not nonstop to the US, so you don't have to get there 3 hours early. Ninety minutes should be sufficient.
I don't have specific hotel recommendations, but there are other active threads about Milan hotels, including many right near Stazione Centrale.

I don't recommend trying to stay at a hotel near the airport since this will sacrifice your last night in Milan. I prefer staying in the city center and enjoying my last night in Europe with a meal, strolling, etc. Even with an early flight it's worth it to pay more on a taxi to the airport in the early morning (your hotel can set this up the night before) than to stay outside the city and be trapped at an airport hotel.
