Suggestions on my itinerary.
Florence to siena to spello to positano to rome to San gimignano to Florence.
When?
Who?
How? Train or car?
April 13 thru approximately may 15 2024.
Myself and my daughter
I've been to italy twice and wanted to visit spello. Want to bring her to places I think she might love.
Probably mostly train but may rent a car from spello area to positano

I’d be tempted to structure it as a round trip from Florence down the ‘western’ part, then back up on the ‘eastern’ part to Florence.
Seems you are planning on flying into and out of Florence.
If your arrival time in Florence allows, I would immediately take the bus to Sienna for the first few days. You will be tired, but Sienna is a beautiful town to be tired in.
See San Gimignano during that time then go to Rome.
From Rome to Positano then back up to Spello and Assisi (see it while you’re so close) then back to Florence for a few days before you fly out.
It is still quite a bit of ground to cover in 4 weeks, but certainly doable.

It isn’t that it isn’t a good idea. I’m mostly thinking of a more efficient trip. However, if you’re thinking you want more time in Rome and Positano, then seeing Spello on the way there isn’t really that difficult. Maybe a bit out of the way, but not difficult. When I plan my trips I look at connecting the dots unless I’m trying to be in one place for something specific. Then I’ll misconnect! If it’s easier to connect to Spello from Siena and then go to Rome, then certainly that’s the way to go. But Assisi is well worth a full day and since you’ll be right there…….
Forum members have many ways of travel planning to save travel time so figuring out the way you want to travel will then make the most sense for you. You will benefit greatly from planning how many nights your want in each place too. That is always the most difficult for me!! Sometimes that and then seeing how long it takes from place to place is a good start.
When you were in Italy/Europe before did you familiarize yourself with train/bus transport there? With few exceptions, we go that route although there are times that renting a car for a few days is more efficient within our time.
In the end, it’s what works best for you and you’ll still have a wonderful trip.
I would consider going to Rome at the end of your trip and flying home from there.

I agree that if you can fly out of Rome or Naples you can do this as a North to South trip rather than looping from Florence to the Amalfi coast and back.
Given the luxury of having that much time I would suggest allocating time for Florence and Rome and then time for the smaller cities you want to see.
I suggest something along these lines:
Florence - 5 days
[rent car]
Siena - day trips to San Gimignano, Volterra other Tuscan cities you want to see 6 days
Spello - day trips to Orvieto, Assissi 5 days
[Return car]
Rome - 1 week
Amalfi coast - 5 days
Probably 1 day lost to airport travel
But priority and timing is obviously up to you. If you're flying out of Rome you can obviously swap Amalfi and Rome.
I wouldn't drive south of Sorrento - Amalfi coast driving is best left to the Italians - but also look into Salerno as a place to access the Amalfi coast via ferry.
So much time is a good problem to have.
