I will be traveling to Rome for 4 nights in February with my wife for our anniversary. Any suggestions on an intinery? I was considering a day trip to one of the Tuscany Hill Towns or Positano but will not be renting a car.
Orvieto is one of our favorites and a quick & easy train ride away. Positano is not really a day trip.

Another vote for Orvieto. It's only an hour from Rome by train.
Positano is too far for a day trip - train to Naples, train from Naples to Sorrento, bus or ferry to Positano, then back. Orvieto would be your best bet.
Richard, a third vote for Orvieto. It's a beautiful Umbrian hill town, with an interesting history.
With only four days in the area, I would probably tend to focus more on Rome and short day trips or tours (Ostia Antica, possibly Tivoli and Hadrian's Villa if you can find a day tour to that site - not sure if all the tour operators will be up to speed in February?).
