Avignonesi does a really nice tour and lunch paired with their wines. It may be technically east of Val D'Orcia proper, about a 20-30 drive from Montepulciano, but close enough that I'd say it's in the region.
They're famous for their Vin Santo, and the highlight of our lunch was the dessert in which they paired 6 different little bites with the vin santo, each one bringing out a different characteristic. And it wasn't officially part of the menu, but they then brought out the Grappa to help us really finish the meal in style! : )
