Do I stay last night by the airport or drive in from Cefalu (where we are staying) in the early morning?
We will have spent 5 days in Palermo on the front end.

I would return the car and spend the last night near the airport. You can’t be sure of traffic conditions so why stress yourself?

What time is your flight and where is it going? Rome?
Have you figured on 2 hours drive from Cefalu, getting gas near the airport, the time finding the rental lot and returning the car (often not just a simple drop-off), and walking from the rental lot to security, plus a cushion for any snafu along the way?
That said, I'd probably do it just to avoid an airport hotel. But we are early risers and not easily stressed. Last April we drove from Sciacca to the Palermo airport (our flight was early afternoon to Rome) in order to stay in Sciacca one last night. You know you best.

Oh I know this is old but having just left Palermo I would say don't take chances with Italian transport when time is a factor! There is a great shuttle bus from the Palermo city center so no need to stay near the airport either.
