I have the same situation with an early flight. I have not stayed there yet, but in my research the Cardano Hotel looked good.

In 2010 we stayed at Best Western Hotel Cavalieri della Corona
It has a free shuttle from 0600 to 2300
Nice rooms and reasonable price--- short ride to terminal ( if memory serves less than 15 minutes)

In late October, in this same situation, I had a very nice room, very affordable rate, and fine overall experience at First Hotel Malpensa.

If the hotel shuttle leaves later than you need, do ask the concierge about private taxi. They are most accommodating. We had made our arrangements prior to leave the US and it worked out great.

I usually stay at the Novotel Ca Grande which is about 30 minutes from downtown Milan, there is a less expensive Ibis next door in the same complex.

If you are flying out of Milan Malpensa airport, then I would recommend the Best Western Jet Hotel. They also have a shuttle to the airport.
