I understand that it is still winter like in mid April, so what suggestions do you have for both inside and outside?

Yes, that is still winter there. As long as you have solid winter driving experience and the weather holds, rent a car and go sightsee. As for what to do, that all depends on how much time you have, your interests, etc.
We leave today for another trip to Iceland and are doing hot springs, snowmobiling, ice cave, northern lights tour, and general sightseeing in a few areas.
Read the threads and see what interests you. Take a look at Iceland With A View as well as her YouTube channel and you will get lots of ideas. We have used her for our last 8 trips there and her advice has always been spot on, FWIW.
