Do I need to print this out?
One for each traveler?
Can I complete it before we travel?

I just printed it out before i left and brought it to airport for my return to the US. The airline might not ask for it, but it's good to have just in case. Each traveler needs one.

NOTE: US citizens do not need to fill out Section 2.
When returning to the US from Germany recently, Lufthansa handed out the forms to each passenger. Upon arrival in the US, no official requested to see the attestation and no one collected them from passengers. I asked both Immigration and Customs officials if they wanted the attestation. No one said they were collecting them and no one asked to see the form. Once home, the paper eventually made it into the recycle container and some other useful purpose.
