Leaving for Greece next month, filling out PLF form today. The instructions say one per household, but my wife and I were vaccinated on different dates with different vaccines. Will it be an issue only having the one form for us?

Your airline might be a better place to ask. I THINK I filled mine out for me and then the next step asked about a second person, so I assume I would have entered that information separately next. However you each have to upload your Vaccination card, so it is probably fine to do the PLR together.

Yes, tdw, you are right and I was unclear. I had to upload my CDC card with United, not with the PLF.

tdw is correct about timing and editing it. I had a flight change 2 days before and had to go back in and edit mine. I also read somewhere that in August, people were getting their email with QR code sooner than midnight - a change in process. But I don’t know that for a fact…..

Our family traveled to Greece recently. My name is different from my husband/son and I was almost denied boarding the plane from Italy. Eventually, the Aegean agent asked his supervisor who agreed it was one form per address for the PLF.
