Individual or Group PLF for Family of 5?

We have been reading conflicting stories about whether our family of five will need to fill out individual PLFs or simply one for the family. My wife and I will be traveling with our three kids, ages 11, 14, and 17. Currently our PLF only lists my wife and me, as you can only add two names. Will this be an issue upon arrival in Athens? Thanks in advance!
