My husband and I will be embarking on a trip to Bavaria soon. We will have D-Tickets, so we'll be using regional trains to get from city to city. We are visiting Munich, Füssen, Augsburg, Nuremberg, and Munich (again). Most of these train rides will be about 2 hours. Are there toilets on the regional trains? Also, are they usually in working condition? (I've been on many a European train, so I won't bother to ask about cleanliness.)
As a best practice go at the hotel or the station before you get on any train; at the train station make sure to have either some coins or a credit card that you can tap before trying to go; train station toilets may require payment to enter usually about 1 euro coin or a tap of the credit card.
All the regional trains that we've used in Germany have had restrooms. Some are cleaner than others but they are there.
All the regional trains we have been on in Germany have toilets. Occasionally one is inoperable. Sometimes we have to go to another car to find one. I don't recall ever having a "desperate" situation on a regional train in Germany. I also try to use the toilet on the train before I get to my destination station so I don't have to pay to use one in the station or hurry to get to my lodging or museum, etc. I have found not all stations have restrooms. Especially the small stations. Large ones always will.

We have been traveling to Germany once or twice every year by train. Recently, the toilets on trains are sometimes out of order, even if there is only one on a small RB train.
Twice (Regensburg and Ingolstadt train stations) the toilets were inoperable and filthy. I wish they would worry a little less about discount fares and more about service. However, Dbahn has to respond to the customer, and most probably worry more about $.
In large stations like Nürnberg we have found that someone takes the toilet fee. In those cases the toilets are operable. I wonder if someone has a concession to provide the service.

All too often, the toilets in train stations are in the basement, down a flight of stairs. I don't mind paying for them (they're usually clean if there's a fee), but negotiating what is usually a long staircase with my luggage is such a pain that I often decide to take a chance and skip that stop.
