Flying in to Frankfurt. We are fully vaccinated. How do we obtain EU Covid green card?

There are a number of posts addressing this and right now, it sounds like it's a bit hit or miss. Some US travelers were able to get the card from select pharmacies in Germany, others had to travel outside the country to get it, and others have just relied on their written vaccine cards.
