I am trying to book my ticket online and there is a note to check the station because we may have to take a bus to Mantes La Jolie. Has anyone tried this recently? Can you explain?

Going from Paris to Rouen you depart from Gare St Lazare. You can take the Metro to reach Gare St Lazare. I've done it both by bus and Metro from Gare du Nord ie, where I stay.
Why not go to the source, which is not RailEurope: https://en.voyages-sncf.c It shows plenty of direct trains although without your travel date it impossible to know if there is might be a disruption to the service. If there is, for instance for maintenance and repairs, a bus will be provided. But you can keep track on the company's site provided above (a better place to buy your ticket too.)Sorry, I posted while you were putting up your date. But the advice holds.
I see that note on Rail Europe's ticket search, but it's odd and I think doesn't ring true.
I don't find anything similar on the French page for "real-time news" at http://www.sncf.com/fr/horaires-info-trafic. You can check that again on the day of departure or before.
Monday is not a popular day for railway repairs. If they can help it, they tend to do them on the weekend. And in those cases, SNCF will normally substitute a free bus departing from the same station, or will publish a timetable for a detour route.
P.S. I'll alert Rail Europe that this looks strange.
There are two stations in Rouen. The trains from Saint-Lazare, as far as I know, all stop at the Rive Droite station. If you have to catch a bus, it could be from there or from the other station south of the river (maybe a mile or so from Rive Droite).
