Do we check the boxes for ‘if I’m not vaccinated’ even though we are triple vaxxed?
What does ‘ Done in………………. Mean? It is followed by the date and signature.
One more question…..do we need to complete the Form from https://passager.serveureos.org/? Or is that for unvaccinated persons….it is unclear since it is under the Unvaccinated paragragh…. And I’m getting bleary-eyed with all the forms.

Ask your airline for guidance, enforcement seems to fall on them.
I assume "done in" is where you would put your location when you're filling out the form.
don't assume - "done in" does indeed need you to fill in where you executed the form.
Done in means the name of the city/place where you filled out and signed the form.
For example, if you live in Houston, Texas, the line will look like
Done in Houston, Texas, United States on 23 December 2021
For the form, go to this official page of the French government (in English), go down the page, click on the tab for “Travel to/from an Amber List country,” and then scroll down PAST the “travel certificates” to the “sworn statement” Word document.

And the boxes that starts with “if I’m not vaccinated….” Even though we are vaccinated we agree to it???? Correct?
There are no boxes to check about “if I’m not vaccinated.” Look at the document I referred you to.
Thanks Kim…..looked again and I see there are no boxes…the one I had printed out earlier definitely had boxes…must have been a different version.
