Hello. I read that to enter France for the US, we need to sign a sworn declaration certificate that I do not have covid. After doing this, do I submit it to someone/place or just take it with me when I fly to CDG? Thank you.

United Airlines has it in their Travel Ready Center as well.
Question: At the bottom of the form, there is room for a date. Do we do it the American way since we are departing from the U.S. or the French way since it is a French form?
I think the way the date is written is not a big deal, you can always clarify if there is a question. But, I would write the date it the European way--many of us do not really remember that Americans do this the opposite way. You can also write, for example, 29 Aug 21 if you want to be entirely clear.
Thanks Azra! I'm not sure why or when the U.S. changed the way dates are written here. You would think it would be done the same as the countries that colonized this country.
