I don't know if you mean 4 star under the French rating system But I'll a shot at some suggestions. Near Place Vendome, I suggest Hotel de la Tamise on Rue d'Alger which is a short walk from Place Vendome. Near Opera/Madeleine, I like Hotel Saint Petersbourg, Astotel Hotel Astra Opera, and Hotel Edouard VII.

Hotel Mansart on Rue des Capucines, just off Rue de la Paix is traditional and intimate or Le Pradey on Rue St Roch for a more boutique/design-y hotel experience.
For a larger hotel, Hotel Westminster on Rue de la Paix.
Wherever you stay, go for the largest room you can afford, if you haven't been to Paris, rooms tend to be small and if it will be two people, you will appreciate the extra space so you aren't bumping into each other, that can put a damper on the trip even if you're only in the room for a short time in the morning and evening.

In early January I spent 4 nights at the Hôtel Astra Opéra, part of the Astotel chain. Its convenient location is a few blocks away from the airport Roissybus stop and also a few blocks from the Opera Metro station. This hotel was a splurge for me at $250 per night, but I thought it was well worth it. Their basement spa area is free of charge to hotel guests and is open from 7 AM to 10 PM. It has a small heated pool, a sauna, and a hammam/steam room. My hotel room was small but very clean. The hotel staff were friendly and helpful. I would stay here again.
