I had better luck to begin with with this link from Trip Advisor. The above link I had trouble being able to sign up for an account and then submit, but it worked with the below link.

I did get an immediate auto response which I did not get when I submitted the "old" way.

I used my US address which is the same I used on the original application. I figured it needed to match my passport (which thankfully it does).
I just went back in to review my submission and am cracking up because they managed to translate my French trader named town (after the Native American tribe) as "Heart of Alene"....so hope that does not present problems!
Heads up for anyone who lives in a French sounding city - Fond du lac? Prairie du Chien? (all I can think of are WI towns, lol)

On another post it says use your French address but to me that makes no sense as it is a French form asking for an American address, I think? They need wikisteps :)

My friend’s daughter arrived in Paris a couple days ago and went to a pharmacy for her Pass Sanitare. Perhaps she got lucky but it was no problem.
Debbie..I just did mine and submitted it. On the drop down list look for États-Unis. That’s the US. Hope this helps!
I put the French address and then at the bottom when it says send a message I said here is my US address, who knows :)
I don't think it will make a difference, but you should use your home residence, not your temporary accommodations in France. The new online form is even more clear as it as follows the "résidence a l'étranger" with another field for "pays de résidence a l'étranger". This can be translated as nation of residence abroad or foreign. It would not be necessary to have this field if they wanted your address in France. (It could also be a routing field used to parse the applications, similar to the different email addresses they used for the old system)

This site is giving error messages when I am trying to upload my documents. I have this in four different browsers (Edge, Chrome, Firefox, and Brave). It either says to check my internet connection (which is fine) or that it cannot store the document. Doc sizes are all in the 200K range, well below their limits, and are either JPG or PDF files.
Anyone else seeing this or have a workaround? Thanks.
So I have started entering my information on the site, and for date of birth it requests the format dd/mm/yyyy. But in the box where you actually enter the date, you have to enter it yyyy/mm/dd! Hope this won't cause problems. Seems like a lot of kinks yet to be worked out on the site.
I just tried to complete the application. In Ontario, Canada we receive a vaccination form for each dose so we don't have a document with both listed. I also have a mix of Covishield / Pfizer. It won't allow me to list both and I can't upload both. Do you think I should just list the Pfizer as my second shot and that vaccination certificate. In notes, should I indicate the first shot is Covishield or just go with the second one and not mention it. Confused!!!
@kathy - Re: 2 documents: My recommendation would be to combine the two documents into one. This could be done by scanning both docs and generating a PDF. If you don't have a scanner at home that can do this, try your public library. At mine, the photocopiers can scan any number of pages, turn them into a PDF, and send it to your email. Librarians are approximately the most helpful people in the world if you have questions.
Hi Kathy, since the site specifically asks for the date of the last injection, I'd provide the vaccine used as the last dose. I'd also recommend the previous poster's advice and scan both receipts into one pdf file. That's what I did.
Thanks. I don't know why I didn't think of doing a PDF of both and I do have a scanner at home. The drop down for what type of vaccine only allows one so i'll choose Pfizer since it's the 2nd dose. Should I make a note at the bottom stating it's a mix?
Kathy, yes, in the box, I'd put in there that you had a mixed dose to call their attention to it.
I had weird things on the birthdate as well - above the box it said DD/MM/YY but in the box it said MM/DD/YY so I went with that.
@dave- I had the same problem last night. I was using my HP laptop and it would not upload the docs . Same error as you, My work around was to save them impn my photos on my ipad and then upload them that way at it worked. We did get an email that our application had been submitted and we have a document number. I did use different email accounts for both of our applications.

@Paqngo Thank you for your reply. I have an Android tablet, copied the files over, and completed the application. I have received the email confirming the submission. I have NO idea why it worked from the tablet and not from the PC, and I was an IT guy for 30+ years.
I did also notice that even though I selected all of my dates from the calendar dropdown for accuracy, it shows them to me in American format (dd-mm-yyyy), rather than the specified EU format (mm-dd-yyyy). I added a note explaining this is what I saw, so hopefully there is no issue. I think this request page got thrown together very quickly and perhaps not tested very thoroughly.

I struggled a bit with the date fields as well. It said DDMMYY, but you couldn't enter a day over 12 so I just entered it in the U.S. fashion. And went back in later in edit mode and the dates were correct in my submission.
I am concerned about the date fields. In all the boxes I have filled out, the date is showing in a YYYY-MM-DD format. I cannot change it, it seems to be the default setting on the site. I am using Chrome for my browser. Anyone else experiencing this?
@GregW - I'd just fill it out starting with the year as indicated in the box and then after you submit you are able to go back in and edit so you can take a look and see if it's right.
@Pam, Ok, thanks. I wasn't aware you could still edit after submitting the form.
I ran into the same issue as others trying to upload jpegs from my laptop. In the end, I completed the non-photo information on my laptop, then logged back in through my iphone and uploaded the jpegs no problem from there. Be sure to size your photoes so they are medium (do not use "small") so as to be under 3mb.
Adresse - I am half French. For the French... "adresse a l'etranger" means your U.S. adress. Remember that the forms were originally intended for French expats living and having gotten vaccinated overseas, before they were adapted to American and other tourists. "Foreign" for the French means "outside of France".
Areas of confusion:
Dates - pay close attention to the format that is *in the box*, not what the text says above it. The birthday box (MM/DD/XXX) directly contradicts the text above it which says "DD/MM/XXXX". The box format (MM/DD/XXXX) is what they want Generally, the format is month/day/year on this form... I guess they tried to "Americanize" it. To be sure, type in the digits into the box, then click the calendar and confirm that is what you wanted. You can also go back into the form and check your dates.... you will then see that the form has converted your "7/12/1983" birthday to "12 July 1983" etc.
Country of Residence - Typing in "USA" or "United states of America etc will not work - the form will reject it. What the form is looking for is "Etats-Unis" and more specifically a word salad which, transalted means
"United states of america on the united states land territory "territiore des etats-unis d'amerique en amerique" as opposed to our overseas territories. This is an artifact of the french administation mindset weher they distinguish between "outre-mer" and "DOM/TOM"... over seas territories vs homelands. This choice can only be accessed via a drop down menu for the box, and I found that the drop-down was accessible from my iphone but not the computer.
When you are done, submit your dossier (Blue button on lower right), and you will almost immediately get a response and an email saying your dossier # xxxxxxxx has been successfully submitted.
The form autosaves as you go, so no worries there about exiting and then going back in. They also allow corrections even after submittal.

Yes, the edit/review function is great. I looked right after I submitted then went back today to review again since I wasn't in the heat of the moment, so to speak, lol. I've bookmarked the address and when you sign in mine takes me right to my "file". Very easy compared to the other method....of course I've not gotten a QR code back yet so we'll have to wait and see but so far much more user-friendly.
Does this mean they are not processing the previously submitted applicatons?
@Janet - It's hard to know and our departure date approaches...so we resubmitted using the new portal.

the new system went into effect yesterday -- we have not received codes from our original application so I used the new system today. One virtue is that is allows you to keep editing the form until it is correct. There are some weird kinks like the form asks for birth date in European order but then actually enters it in US order. I had trouble getting the country to load on the form -- it is etats unis and not the English version although much of the form is in English.
We got notice when the account was set up and then for each of the applications made through that account, so didn't have to use separate email addresses. The old system required someone to type an email off the application which allowed errors to creep in. This form will presumably respond to the email and it permits multiple applications on the account.
And now we wait and see how it goes.
Sorry if this is a dumb question, but on the new demarches application, when I use the drop-down mention to select which vaccine I have, I only see vaccines with EU on the end of them: Pfizer-BioNTech COVID-19 vaccine EU/1/20/1528.
Is this what I select or did I end up on the wrong form somehow?

@CaliMom, There were 2 Modernas to pick from and I just chose one of them. There is a box toward the end where you can add a note so if you want to explain there you can.

I applied under the old email system on 8/24, no reply until today.
Where I got an email stating that they are receiving too many requests and will not get to all of them... Ok....
That email directed me to the new online form, so I take it that they may not be processing older emailed applications anymore.
I actually had already re-applied using the new online form, on September 1st.
Still waiting, but we are not arriving in France until the 21st.
I got the same email yesterday as well. And like you, had already reapplied through the website. Still in "en construction" status as of today, meaning nobody has looked at the application.
I am totally disgusted with the whole thing at this point and am considering just cancelling everything and getting a refund from Air France. It's ridiculous if one thinks about it. They demand that you comply with their system, but don't process your request.
At least half of my hotel bookings state in the confirmation email that you must present a Pass Sanitaire to access their property. One even went so far as to precie that "only a eurpoean health pass" would be accepted.
I will email the hotels and get assurance from them that they accept our CDC cards as the pass sanitaire. That will be my last effort to make this work. There are other places, including here in our country, that I can spend my tourist dollars without this kind of grief.
@possumracing: I feel your frustration, but our first week here we didn’t have our application approved, so we went and got tested every three days. Is it a hassle? Yes. Does it work? Absolutely— the antigen tests here give you the same QR code you can use in the app. Is it worth it? A THOUSAND TIMES YES!! You get to be in France, enjoying life and eating and drinking and feeling a part of France.
There is no place in the US that can offer the enjoyment and beauty of France, so even if you don’t get the QR code before coming, you can easily make it work!!

goodness! I'm so glad I found this thread - the date labels definitely led me to input our birth dates incorrectly (days and months swapped), just found them when checking and was able to modify. Thank you!
The birthdate has a drop down calendar on the site -- so I picked like the month of november and it shows that month and you select the date --- it then enters it in US format. It just seems like if you use THEIR drop down system it ought to work.
