EU digital Passenger Location form

I am really confused.
My airline says to enter France I can print out a passenger locator form and fill it out.
But I see in other places that this has to be done digitally on this website.

Has someone recently been to France and knows first hand what exactly they require?

Not going until mid April, but I filled out the form online, downloaded the PDF (they send a link) and will print it out. One less airport holdup (I hope).

We flew into Paris CDG on Delta last week. And I’m still confused. : )

We were unable to get into FlyReady do anything online for check in so we just showed up at the Seattle airport about 3.5 hours before our flight (more than enough time). The agent checked our CDC vax cards, health attestation (that we had printed and filled out at home) and passports and gave us the link you have and had us do the online locator on our phone before she would complete our check in and issue boarding passes. On the plane the flight attendants passed out paper locators that we diligently filled out - probably didn’t need to. : )

At immigration/passport control in Paris they only wanted to see our passports. Hope that helps.

I can second the previous post. Just arrived in Paris this morning. I did the electronic version of the locator form and saved the PDF on my phone.

Delta agent asked to see it when we checked in, but no one else seemed to care. They were passing around a French version of the plane, but we did not complete it. No one asked to see it upon entering France.

Can you send a link to the health attestation. I am flying on American and there is no form for that.
Just a pdf for the passenger locator form.

Thanks for the link, didan. I had filled out most of the form when I noticed they want my Social Security number. I'm not comfortable giving this out. The site seems to indicate that filling it out and submitting it will generate a form which means there is a record of this someplace and who knows who has access to it. Do you know if this is really necessary? Has anybody else had reservations about supplying their Social Security number?

We leave for Lyon in 3 weeks and in Delta Fly Ready no mention of health attestation form only locator form which we have done and printed out. I thought I read in this forum last month that that was no longer needed. Is that correct?

Do not put your Social security number.
Fill it with zeros or leave it blank.

I have looked on the US embassy site and the French interior ministry site and it says the US is considered a green country and so that means you do not need to fill out the health form only the locator form. I think this is why that form is no longer on the delta Fly Ready site.

I flew to Paris from Pittsburgh via Atlanta on March 18. I was unable to upload anything to Verifly before I left. I didn’t try very hard because I knew other people had problems with it.

At the Pittsburgh airport, the Delta rep asked me for my passport. After poking around on her screen for a while, she asked if I had filled out any form. I said I didn’t think one was necessary. (I learned this from Kim on this forum.) She never asked me for my vaccine card. I was not asked for anything in Atlanta besides my passport and boarding pass. While on the plane, they passed out a passenger locator form and collected it before we landed. At immigration in Paris, they only asked for my passport. That was my experience.

THE HEALTH ATTESTATION IS NO LONGER REQUIRED.

that is why an airline wouldn't be asking for it now.

We leave Next Tuesday. Over the weekend, we logged into our Delta Acts, and attached PDF of our CDC Vaccine cards, Passports and filled out the EU form per the link from Delta. We created PDF files of our EU Locator forms, and attached as well (of course we will have printouts of everything with us.). We are all set - better to be prepared for worst case scenario than not :-).

@Kay: The SSN on that form is optional. I skipped it.

Kim points out the attestation no longer appears to be necessary but, having created one and printed it I'm taking it. Because why not, the way things change!
