If arriving at terminal 5, you can but it at the stand just before going down to the tube. Believe the stand is also for the Heathrow Express. You can also buy them at the kiosks at the tube stations.

Bought ours at the TI in the airport. I think it is somewhere near the tube. Just don't remember.

Keep following the signs to the underground station. It was a maze due to construction when we were there last Spring. Once you are there, there are machines as well as a human you can buy them from. Easy.
