In 2025 I would like to go to Montenegro, Croatia, Bosnia-Herzegovina, Serbia, Slovenia. Out of the three choices below which date would be the best and why?
Looking for warm temperatures to swim and enjoy the sites but not oppressive heat. Have heard Slovenia can be rainy in October.

I would choose the first one for best guarantee of swimming temps. Maybe second if you are really worried about heat.
Two weeks is pretty tight for five countries though, I'd scale back.
Fall can arrive during the second half of September, meaning more rain. I ran into some downpours in both Montenegro (late September) and Zagreb (early October). It shouldn't be cold down at sea level. I don't go to beaches, so I can't say anything about the water temperature.
I agree that you're trying to cover an aggressively large area in just two weeks. If you're not planning to travel by rental car (if you are, be sure you have permission to take the car into all those countries), have you dug into ground-transportation schedules? Trains are pretty rare and are not fast; buses are not fast; cross-border transportation of any type is infrequent.
