Bulgaria -May reservations? Medical?

Going a 10-day tour, then thinking about extending for a week or two on our own, going where our fancy takes us.

Do you think lodging reservations would need to be made in advance, or just day of or day before in mid-late May? Specific towns that might need reservations?
Anyone have a medical emergency in Bulgaria? Wondering about the quality of their providers and facilities. (We do have medical evacuation and some coverage out of the country too.)
Are the villages along the Black Sea unique enough to warrant visiting in the off season?

For the coast I cant speak to villages. I am guessing you have a car so aren’t relying on a professional who will know such things and the hidden off the beaten path stops of interest. For the Black Sea cities, I’m not sure May is off season. Burgas, Varna and Nesebar I imagine will be geared up and ready for tourism.

For medical, yes, traveling without the guidance of a local makes such questions more important. I would research private clinics that cater to tourists at each of your stops and keep a list. Ive never had the need in Bulgaria but other Central & Eastern European countries yes and I prefer private to their system care.

Hotels? They will all take reservations the same day except for the ones that are fully booked. I suspect that dynamic pricing has made its way to Bulgaria so costs might be unpredictable. Have you made a list or are you going to wing it when you get there? Book now but cancelable would be my suggestion.
