Do i need a covid test, when i only have a transit in the USA. I am also fully vaccinated.

Hold on here, I think there is a communication issue.
Are you talking about arriving by air to the United States and then changing planes to another country? If so, I think the answer is “yes”.
Can you explain in a bit more detail what your plans are?
I also think you should check with your airline. They will make the decision whether you board your flight to the US or not.

I fly from Medellin to Miami, where I have an 8-hour layover and then fly on to Kingston (Jamaica) with another airline.

I think you will need a Covid test and also an ESTA for the United States (unless you are US citizen) as you will enter the United States officially when you arrive in Miami. You will go through immigration/customs and then have to check in for your next flight.
Once again, you should be talking to your airline about what they expect for you to board that plane.
