I have a trip planned in October to Egypt and Jordan with a tour company and I plan to add 5 extra days in Jerusalem after the tour. I'll be staying in either the 3 Arches hotel or the Notre Dame Hotel (Haven't narrowed it down yet) just outside of the old city. I would like to take some guided tours of the holy land, West Bank, Bethlehem, etc. Can anybody recommend some good options?

Green Olive tours has a good reputation for dual narrative tours in that area. Medji tours as well.
We used Guided Tours Israel for a three day tour for a cruise group. They worked with us to modify one day. They are highly rated by folks on Cruise Critic. We were very happy with their services.
I used Rent-A-Guide for tours. No complaints.
It has been a few years, but we used Ashraf Shaheen ([email protected]) for West Bank/Bethlehem/Jericho tour (1/2 day). He was highly recommended and fantastic. We booked him through Abraham Hostel in Jerusalem, which offers many tours (private and group), but you can also book him directly. We also did a private tour with Dvir Hollander ([email protected]) while in Jerusalem. He was wonderful and can tailor the tour to your interests.
I second Green Olive Tours---very educational, not so touristy. You will get a good sense of both sides of the Israeli/Palestinian situation. We've went with them on 3 different tours, including Hebron & Nablus, which some say are too dangerous to travel to, and Ramallah. The Tomb of the Patriarchs was the highlight-----and Jacob's Well. Remember to get some knaffeh in Nablus---the sweet treat originated there.
