Planning a trip at the end of May, will visit Brussels, Bruges, Ghent, possibly Antwerp, and Strasbourg, Colmar, Alsace Wine Road.
Is it better to start in Brussels and end in Strasbourg, or vice versa? Also, if we have an extra day, should we spend it in Brussels or Strasbourg?

Make sure you stay in one of the wine villages after staying in Strasbourg.
As Suki says, Definitely get to one of the medieval wine villages in the valley for a couple nights ( Riquewihr, Ribeauville, Eguisheim, ) Strasbourg is also well-worth a couple nights. In Ribeauville, there are castles, a wine co-op where you can sample wines from multiple vintners all under one roof, and a town of half-timbered houses that will take you back 500 years. Rent a bike and ride the dedicated bike trails to several medieval towns through miles of vineyards.
A few years ago, we started in Paris, took a TGV train to Strasbourg and stayed in Ribeauville and then Strasbourg which has its own small airport ( SXB) from where you can fly to Brussels ( BRU) or Amsterdam. If you pre-buy your train tix and flights now, you can get great fares.
